FAMILIES AND STUDENTS Agree to The Following:
Parents/ Guardians are asked to be on the alert for signs of illness in their children and to keep them home when they are sick. If a child is to be kept home due to illness, please contact Robertha Hernandez at 415-240-3977.
Parents agree to report any COVID infections to CICLO SF staff immediately.
CICLO SF CONTINGENCY PLAN:
CICLO will notify students, parents, SFYT staff, management of possible COVID-19 exposure and
class postponement.
In such a circumstance, we will maintain confidentiality of the student or staff member with
confirmed COVID as required by the Americans with Disabilities Act, the Family Education
Rights and Privacy Act, and possibly HIPAA.

I understand the risks associated with enrolling my child in CICLO afterschool classes, and Day
camps and agree to assume the risks to my child and my household. I also agree to follow all
safety requirements that the program imposes as a condition of enrolling my child.
